These are the items you're thrilled to roll. They either delete opponents outright or shift a fight so hard that a mediocre partner item stops mattering. Several are VIP-tied, which is why they're rare in public games but dominant when they appear.
Death Note is the strongest item in the game, full stop. It instantly kills whatever it targets — not just players but bosses too — and it even ignores the Franklin Badge, an item that would otherwise block an incoming hit. The only limit is that it's disposable: it's a single-use kill and then it's gone. It's a VIP-tied item, so you'll mostly see it in VIP servers or through Borrow rather than a lucky public roll. When it lands in your bar, it's a guaranteed pick-off with no counterplay.
Dragon Deagle is the best repeatable damage item in Item Asylum. It fires roughly 20 times per second and deals 99,999 damage per hit, which is the highest damage output of any non-instakill item in the game. Unlike the Death Note, it doesn't disappear after one use, so as long as you keep it you're a walking delete button. It's another rare, VIP-associated roll, and it's the item most veteran players name when the conversation is about anything short of a true instakill.

It's a ranged-slot roll, so pair it with any decent misc item and you've got a loadout that wins most engagements at the range shotguns love. If you roll it, don't rush melee range — bully the mid distance where it shreds.
A-tier items are only a notch below the meta breakers. Each one can carry a round on its own, and the gap to S tier is usually a windup, a reload, or the fact that it doesn't ignore defenses the way the top four do.
Gatling Gun (the minigun) is a raw-output monster in the ranged slot. Its rate of fire is insanely fast and its effective range isn't bad either, so it pins targets and melts them once you're on aim. It doesn't reach Dragon Deagle numbers and it's easier to interrupt, but for a rollable ranged item it's about as reliable as sustained damage gets. Hold a lane, keep the stream on a target, and it does the rest.
Necromancer's Scythe is the standout scaling melee. Its three-hit combo deals about 22.5 damage per swing with stun and poison, landing roughly 76.5 total across the full combo including poison, and the final swing ragdolls whoever it hits. The real payoff is the soul counter: each player kill adds 1 soul (NPCs add 0.5) up to 10, and every full soul grants +15 max HP, up to a massive +150 maximum health. Snowball a few kills and you become extremely hard to put down. The catch is that it needs those early kills to reach its ceiling.
Fire Axe is a strong melee in its own right and the other half of the best non-VIP combo in the game. Solo, it's a dependable heavy swing; paired with Ultra Instinct's synergy and bonus health pool, it becomes a serious duel-winner. Because it depends on a two-item roll to hit its true peak, it sits in A rather than S — but the moment you have both slots lined up, it plays like an S-tier package.
Sniper Rifle is the roster's long-range threat. It warns the target, then after a roughly 4-second delay fires a bullet for a high 75 damage, and it can fire up to 3 bullets until the target leaves range, hides behind a wall, or the shots run out. The telegraph means good players can juke it, so it's not a blind win, but from across the map it applies pressure nothing else in a normal roll can match. Use cover and angles to make that 4-second window unavoidable.
B-tier items are genuinely fine rolls. They'll win fights at every skill level, but each trails the picks above in consistency, range, or ceiling.
Slayer's Sword is a massive greatsword that swings in a wide circular arc, making it strong against groups and easy to connect with. Poser's Katana is a stylish melee that's good but a little overhyped. In the right combo it's a strong opener, but the delayed payoff and setup requirement keep it out of the top tiers. Treat it as a control tool that leads into your damage, not the damage itself.
C-tier items aren't broken, they're just outclassed or niche. You can absolutely win with them, but most players get more from the rolls above.
Golf Club is a functional melee that the community rates below the likes of Slayer's Sword and Necromancer's Scythe. It swings, it knocks, and it can finish a weakened target, but it lacks the range, scaling, or utility that makes the better melees worth building around. If it's your slot-1 roll, lean on your ranged and misc items to carry the fight.
Board with Nail looks the part but suffers from a janky hitbox that misses hits it visually should land. When it connects it's a passable improvised melee, but the unreliable hit detection means you'll whiff trades you thought you won. It's a fine emergency swing and a poor plan A — hence a firmly situational placement.
Anti-Material Sniper Rifle is a specialist. Its .50-caliber round posts the highest damage against hostile NPCs and players of the sniper family, so against bosses and PvE targets it's genuinely excellent. The problem is the roughly 10-second reload, which leaves you exposed and useless between shots in the fast, chaotic PvP that defines most Item Asylum rounds.
